Our office is in-network exclusively with Delta Dental, as we are a Delta Dental Premier provider. We are considered out-of-network with all other dental insurance plans. However, we are happy to submit claims to your insurance on your behalf and assist you in maximizing any available out-of-network benefits. Our team is always available to help you understand your coverage and provide an estimate of benefits prior to treatment.

- Completed Health History.
- Signed Privacy Acknowledgement
- Dental Insurance Card (if any).
